Fill this form when a breakdown is reported from the field.
Steps
- Pick the casualty from the register if it is known. If it is not
registered, just describe it (for example "Bedford truck, gearbox
failure"); it can be registered properly when it arrives.
- Give the location as precisely as you can, with a grid reference
if you have one.
- Record who reported it, their unit, and what is wrong.
- Set the priority and tap Log recovery job.
The system issues the job number (RV-...) itself. The supervisor then
dispatches a recovery team from the job page.
